Background. Hemophilia causes recurrent joint bleeding, leading to chronic arthropathy and impaired quality of life. Despite major therapeutic advances, effective management requires sustained self-monitoring and longterm adherence. Digital health tools, including telemedicine, mobile applications, and electronic diaries, may enhance self-management, improve data quality, and support personalized, data-informed clinical care.
Aims. To describe the Tight control of treatment adherence and efficacy by tElemedicine for an improved personalized Management of Patients with hemOphilia (TEMPO) platform, an Android application designed to support patients on prophylaxis through comprehensive treatment tracking, event documentation, and physical activity monitoring.
Methods. TEMPO provides an electronic diary enabling patients to record bleeding episodes and pain events, including anatomical location, cause (spontaneous, post-traumatic, or post-procedural), and pain intensity. The application features a flexible prophylaxis scheduling system with customizable reminders, allowing users to confirm or postpone infusions and to document on-demand treatments with detailed medication information (e.g., drug name and dosage). The application integrates physical activity monitoring through Health Connect (https://developer.android.com/health-and-fitness/health-connect/), collecting data such as daily steps from compatible devices. Climate conditions are recorded as well, without recording location, to preserve the patient’s privacy. A subset of patients participates in an extended monitoring protocol using Movesense (https://www.movesense.com/product/movesensesensor-hr/) wearable sensors for inertial data collection during physical activities. All data are synchronized to a secure cloud database and can be exported as CSV files, enabling patients to share information with healthcare providers during clinical visits. A dedicated dashboard for real-time physician follow-up has also been developed.
Results. TEMPO is currently available on the Google Play Store and is undergoing evaluation in a real-world clinical setting involving 25 patients. Preliminary feedback indicates high usability, improved treatment adherence, and increased self-awareness of personal health status. The application supports both Italian and English and is designed for seamless integration into routine clinical practice.
Conclusions. TEMPO aims to overcome the limitations of traditional paper diaries by providing real-time, structured data collection and automated reminders. This digital approach has the potential to improve treatment adherence, reduce recall bias in event reporting, and strengthen patient-clinician communication. In the future, TEMPO-generated data may support physicians and patients in shaping increasingly personalized management strategies for hemophilia.
